Critique of the extracranial-intracranial bypass study

Abstract

The potential benefit of extracranial-intracranial anastomosis was evaluated by a multicenter international cooperative study headed by the group in London, Ontario, Canada. The final conclusion of the study was that the extracranial-intracranial anastomosis did not provide any benefit over the treatment with aspirin. Several objections and shortcomings have been identified in this study. In our evaluation it cannot be concluded that EC-IC bypass surgery is not effective in reducing stroke in all patient populations.

      Surgical revascularization using STA-MCA bypass surgery failed to prove efficacy for internal carotid artery occlusive disease compared with medial therapy in the EC/IC Bypass Study.5 However, the study was criticized because of the large number of patients who were operated on outside of the study and the lack of hemodynamic assessment during patient selection.23,24 After that, 2 randomized controlled trials (RCTs) were conducted to prove the superiority of STA-MCA bypass applied for restricted patients who showed obvious hemodynamic impairment with objective indicators.11,25

      In 1969, Yasargil111 described the superficial temporal artery–middle cerebral artery bypass for the management of complex aneurysms, which redirected extracranial flow to the intracranial circulation with a single end-to-side anastomosis. Despite the negative results of the Carotid Occlusion Surgery Study, this technique has been widely adopted for complex aneurysms that cannot be managed with conventional microsurgical or endovascular techniques.112,113 In response to problems with limited flow, a second-generation technique was subsequently developed using high-flow interposition grafts connected to donor sites in the patient's neck.114-129
